eRate Support Services Agreements

All districts need to complete the following forms:

• Form 479
• Letter of Agency-ESU 10
• Letter of Agency-State of Nebraska

If you currently have an Erate Support Services Agreement that will expire on August 1st or are interested in that service; complete the support aggreement and indemnification agreement below:

• Erate Support Agreement 2012-2013
• Erate Indemnification Agreement  -This form must be signed and notarized
